Who this is for

Have overweight or obesity.

Have a parent or sibling with type 2 diabetes.

Are physically active less than 3 times a week.

Have non-alcoholic fatty liver disease (NAFLD).

Have ever had gestational diabetes (diabetes during pregnancy) or given birth to a baby who weighed over 9 pounds.

Are an African American, Hispanic or Latino, American Indian, or Alaska Native person. Some Pacific Islander people and Asian American people also have a higher risk.

Were told by your healthcare provider that you are at risk for Prediabetes

What's included

A CDC-approved curriculum with lessons, handouts, and other resources to help you make healthy changes.

A Lifestyle Coach, specially trained to lead the program, to help you learn new skills, encourage you to set and meet goals, and keep you motivated. The coach will also facilitate discussions and help make the program fun and engaging.

A review and check-in on weekly progress, including a private weigh-in.

A discussion about the week's topic that will give you a chance to learn from the program materials and each other.

A wrap-up with a to-do list and handouts to reinforce what you've learned.

What is online health coaching?

Online health coaching is a personalized, supportive process that helps you improve your health and wellbeing through goal-setting, education, accountability, and behavior change—delivered virtually via video calls, phone, or messaging.

Who is this coaching for?

Button

This coaching is suitable for adults who want support with areas such as: Being told by a healthcare provider that you are at risk of developing Type 2 Diabetes Being overweight Being 45 years or older Having a parent, brother, or sister with type 2 diabetes Being physically active less than 3 times a week Ever having gestational diabetes (diabetes during pregnancy) Giving birth to a baby who weighed more than 9 pounds Having polycystic ovary syndrome

How are the sessions conducted?

Sessions are conducted online using a HIPAA secure video conferencing platforms (such as Zoom or Google Meet). You’ll receive a link before each session.

How long is each session?

Most sessions last 30–45 minutes

What do I need to participate?

Button

A stable internet connection A smartphone, tablet, or computer with a camera and microphone A quiet, private space for the session To participate, you must meet ALL 4 of these requirements: Be 18 years or older. Have a body mass index (BMI) of 25 or higher (23 or higher if you're an Asian American person). Not be previously diagnosed with type 1 or type 2 diabetes. Not be pregnant. You'll also need to meet 1 of these requirements: 1. Had a blood test result in the prediabetes range within the past year (includes any of these tests and results): Hemoglobin A1C: 5.7%–6.4% Fasting plasma glucose: 100–125 mg/dL 2-hour plasma glucose (after a 75g glucose load): 140–199 mg/dL. 2. Be previously diagnosed with gestational diabetes (diabetes during pregnancy). 3. Received a high-risk result (score of 5 or higher) on the Prediabetes Risk Test.

Is my information kept confidential?

Yes. All personal information and discussions during coaching sessions are kept confidential and handled in accordance with privacy and data protection standards.

Is this coaching medical treatment?

Button

No. Health coaching does not diagnose, treat, or prescribe medication. If you have a medical condition, you should continue working with your healthcare provider. Coaches may support lifestyle changes that complement medical care.

How should I prepare for my first session?

Before your first session: Reflect on your health goals and challenges Complete any intake forms sent to you Be open and honest—there are no “right” or “wrong” answers

What if I need to cancel or reschedule?

Button

If you need to cancel or reschedule, please do so at least 24 hours in advance to avoid losing the session. Refer to the program’s cancellation policy for details.

How often will we meet?

Button

This depends on the program. Sessions may be weekly, biweekly, or monthly. Your coach will recommend a schedule that best supports your goals.
